Is it balanitis or syphilis?

Patient's question:

After unclean sexual contact, around 20 days later, I noticed several red spots on the glans and 3 to 4 on the coronal sulcus, with intense itching. Worried about a sexually transmitted disease, I went to the People's Liberation Army Hospital for a thorough examination. At that time, I had a blood test for syphilis. The result was negative. The doctor said it was balanoposthitis, with a large amount of fungi found in the prostate. Then, I had a circumcision surgery, where the doctor used a laser to remove the spots on the glans, but only 3 to 4 spots on the coronal sulcus were removed.

Doctor's answer:

The incubation period of syphilis is approximately three weeks. If you carefully check your blood and find that syphilis is negative, then you can rest assured. After contracting syphilis, there are no obvious symptoms in the first stage, and the patient will not feel anything at all. Based on the symptoms you described, it aligns with balanoposthitis. It is recommended that you focus on systemic treatment while also coordinating local treatment. If the balanoposthitis is caused by a fungal infection, it is advised that your sexual partner also gets checked to see if they have been infected as well. The best approach is for both partners to be treated simultaneously.
