Enlargement of unilateral epididymal mass with occasional discomfort

Patient's question:

Last October, I had epididymitis. During treatment, I had an ultrasound, which showed no abnormalities. After taking antibiotics, it mostly healed, but there was still a hard lump the size of a peanut kernel. Recently, I noticed the lump had grown to the size of a thumbnail, and occasionally I felt a slight sour or aching sensation.

Doctor's answer:

According to your description, the testicles are connected to the scrotum through a tissue called the gubernaculum, which fixes the testicles to the scrotum. In some fetuses, during growth and development, one or both sides of the gubernaculum may become excessively long, resulting in high mobility of the testicles and spermatic cord. If sudden exertion or severe violent shocks occur, the testicles and spermatic cord may undergo a 360-degree or greater rotation. Based on the symptoms described, it is highly likely that the diagnosis could be testicular torsion, orchitis, or epididymal cyst. Further diagnosis is required after hospitalization, combining the results of a physical examination and relevant tests to clarify the condition in detail. It is recommended to undergo regular check-ups and treatment. During the treatment period, sexual intercourse is strictly prohibited.
