Small, asymptomatic cysts generally do not require treatment and will never affect fertility. If the condition worsens and the cyst shrinks, and there is no recommendation for conception, surgical treatment may be considered, as surgery may damage the epididymal ducts and affect fertility. In general, the possibility of malignant transformation is low.
