An epididymal cyst may be caused by obstruction of the epididymal ducts due to local inflammatory adhesions, or by fluid retention within the lumen after obstruction of the epididymal vas deferens, leading to the formation of a cyst. Surgical removal is feasible for patients with severe symptoms or large cysts, as well as for unmarried individuals with children. However, surgery may damage the epididymal ducts and affect fertility, so caution is advised for married patients. An epididymal cyst is a small, hard, smooth lump in the epididymis containing yellow fluid, but it may feel firm yet soft upon palpation. It can appear on one or both sides and vary in size. Epididymal cysts tend to recur.
