For men with low sperm count caused by undescended testicles, the issue of undescended testicles should be addressed early. It is generally recommended that orchiopexy (surgical descent of the testicle) be performed during early childhood, ideally before the age of 28, and no later than 68. In adult males with undescended testicles, if the other testicle is normal, the affected testicle can be removed to prevent future complications. Preserving one testicle does not affect sexual function or fertility. For varicocele, the best treatment is high ligation of the spermatic vein. Approximately 50% to 80% of patients may see their sperm count return to normal within about 18 months after surgery, enabling their wives to conceive. The severity of varicocele does not have a proportional impact on sperm quantity and quality; sometimes mild symptoms can also lead to significant changes in sperm.
