For more severe cases, high ligation of the spermatic vein can be performed, which involves ligating all branches of the spermatic veins at the internal inguinal ring. The current best surgical approach is laparoscopic high ligation of the spermatic vein. Varicocele tends to recur, requiring long-term medication.
