Is this normal balanitis or herpes?

Patient's question:

Seeking advice, is this ordinary balanitis, or trichomoniasis of the prepuce and glans, or genital herpes? The prepuce has been excessively long, with benign pearly papules, and occasionally a few small red spots on the glans, but they do not persist. My wife has recently had yellow and excessive leukorrhea, and a hospital check revealed trichomoniasis of the vagina. She was told that both partners need treatment. The medication prescribed (tinidazole) is a carcinogen, and I'm not very keen on taking it. Today, while taking a bath, I specifically examined the inner side of the prepuce and found some small red spots, apart from a few tiny blisters (the larger ones are not blisters, but blood vessels), which are painless and itchy. Could it be genital herpes, or just ordinary balanitis?

Doctor's answer:

Trichomoniasis is a condition that requires treatment for both partners. Trichomoniasis can be transmitted orally, and tinidazole can be taken orally. It is recommended not to be overly anxious. Small blisters on the glans can be treated by a surgeon. Pay attention to personal hygiene, engage in appropriate activities, and boost your immune system. The condition of balanitis tends to recur, requiring long-term oral medication. For male patients, it is important to seek treatment from a professional and reputable hospital promptly. Treatment should be considered based on the results of a thorough examination. Additionally, maintain a proper diet and avoid sexual activity.
