What are the causes of epididymal malformation?

Patient's question:

The epididymis is combined with the testis, and the epididymis appears as a long loop, significantly longer compared to the testis. The markedly elongated epididymis is abnormally adhered to the testis. Lower back pain and dull pain, relieved by rest, worsened after exertion. What are the causes of epididymal malformation?

Doctor's answer:

If the mesonephric duct fails to develop at all, it can lead to epididymal malformation. If development ceases at a certain location, it results in self-locking at that site. When the epididymal duct coils around an obstacle, it can again cause significant shortening of the epididymis, leading to a recurrent loop-shaped epididymal malformation. Epididymal malformation is relatively common in clinical diagnosis, typically referring to marked elongation of the epididymis or abnormal adhesion to the testis. Epididymal malformations include structural abnormalities such as epididymal agenesis, segmental self-locking, and epididymal cysts. Generally, there are no clinical diagnostic symptoms, and they are often discovered during male infertility investigations or orchiopexy surgery. They are frequently associated with vas deferens malformation.
