Can a semen test replace a prostate exam?

Patient's question:

I feel like I have prostatitis. I had a urinalysis, blood test, and a color Doppler ultrasound of the urinary system, all of which were normal. Then the doctor didn't ask me to do a prostatic fluid test but instead recommended a semen analysis. During the semen analysis, he also checked for white blood cells, and everything was normal. He said I was fine. Can this semen analysis replace the prostatic fluid test?

Doctor's answer:

Normal prostatic fluid is translucent, without sperm or with very few sperm, and is expelled after sexual activity has begun.
It is recommended that semen tests generally cannot replace a thorough examination of prostatic fluid. If the prostatic fluid examination is indeed normal, there is no problem. If you feel discomfort, you can apply cold compresses.
For patients with prostatitis, timely treatment can reduce the risk of recurrence. If left untreated, it may affect fertility. Therefore, patients should seek timely symptomatic treatment and pay attention to self-care in daily life, avoiding spicy foods.
