At week 6 of embryonic development, the mesonephric duct and the paramesonephric duct are formed. These ducts will differentiate into the male and female reproductive tracts. After the embryonic gonads differentiate into testes and produce testosterone, the mesonephric duct gradually differentiates into the male perinatal duct under the influence of androgens. The cranial part of the mesonephric duct becomes the appendage of the epididymis. The testicular efferent ductules, derived from the mesonephric tubules, along with the epididymal duct, which winds and coils around the underlying mesonephric duct, together form the head of the epididymis. The remaining epididymal ducts form the body and tail of the epididymis. The etiology of congenital epididymal malformations is still unclear.
