Why does the upper part of the ointment pill become coarse?

Patient's question:

I am currently 20 years old. This year, I noticed that the upper part of the seminal vesicle (it should be the epididymis or spermatic cord) tissue has become thicker and larger in size. It can be felt more noticeably than before. Generally, there is no pain, but there have been two or three instances of unclear cause pain, accompanied by a slightly swollen sensation. I visited a doctor who diagnosed it as prostatitis, but the medication didn't help. The swelling hasn't subsided. Now it doesn't hurt, but I'm worried it might be a serious condition. I want to know why this is happening?

Doctor's answer:

Swelling in the upper testicular tissue, after treatment, proved ineffective. Based on your description, the preliminary suspicion is varicocele. Generally, there are no symptoms, but when symptoms do occur, they include scrotal fullness and a sensation of heaviness, groin pain, and relief when walking or standing for a long time, as well as alleviation or disappearance after lying down and resting. If the varicocele does not disappear when lying down, it may be secondary, and the cause should be identified. Treatment: For those without symptoms or with mild symptoms, elastic underwear or a scrotal support can be worn. Because varicocele can interfere with sperm production and semen quality.
