The patient, due to urethral stricture, experiences bladder sphincter contraction during urination. Before or simultaneously with overcoming urethral resistance, the intravesical pressure drops below the tolerance level of the bladder's tunica muscularis. Urine refluxes into the ureters, causing dilatation of the ureters and renal pelvis, leading to upper urinary tract bacterial infection, scar formation, and ultimately secondary reflux nephropathy or even kidney function damage. In cases of phimosis and paraphimosis, chronic irritation from urine and smegma can result in edema, congestion, and desquamation of the prepuce and glans mucosa, followed by repeated cross-infection and even reoccurrence of phimosis entrapment.
