Why does varicocele cause testicular pain

Patient's question:

It seems like there are things like worms in my testicles, and sometimes it's very painful. Should I have surgery immediately? Sperm motility is also low. What should I do?

Doctor's answer:

Hello, symptoms of varicocele:
1. The patient may be completely asymptomatic and only discovered during a physical examination.
2. The affected scrotum or testicle may have a sensation of heaviness, fullness, or pain. The scrotum may become enlarged, with the affected side being higher than the healthy side when standing. The surface of the scrotum shows dilated, tortuous veins. A soft, worm-like mass may be felt upon palpation, and symptoms may reduce or disappear when lying down.
3. The patient may experience neurotic symptoms, such as headaches, fatigue, or hyperesthesia. Some patients may have sexual dysfunction. Mild varicocele does not require treatment.
