How should varicocele be treated?

Patient's question:

I am a taxi driver and have been sitting in a small space for a long time, lacking in physical activity. Recently, I noticed some pain in my testicles, and they seem to be of uneven height. Looking at the symptoms, I did some research and suspect it might be varicocele. I would like to ask: What should be the treatment for varicocele?

Doctor's answer:

Varicocele can lead to male infertility through various different pathways. It not only interferes with the regeneration of sperm but also causes an increase in sperm vitality. Therefore, patients with severe varicocele still need to consider active treatment. For mild varicocele or those accompanied by neurasthenia, measures such as reporting the scrotum and applying cold compresses are recommended. For severe varicocele, continuous sperm count below 20 million three times in a row, or testicular atrophy; if the varicose veins disappear when lying flat, the varicocelectomy with high ligation of the internal spermatic vein is a viable option.
