Common causes of orchitis include approximately 20% of mumps patients developing secondary neuritis of the testes. This condition is more prevalent in the later stages of puberty. Macroscopically, the testes are highly swollen and appear purplish-blue. When the testes are incised, due to interstitial reaction and edema, the testes cannot be expelled. Histological examination reveals edema and vascular expansion, with extensive infiltration of inflammatory cells. Spermatogenic tubules show varying degrees of degeneration. During the healing phase of orchitis, the testes become smaller and softer. Spermatogenic tubules are severely dilated, but the interstitial cells are retained, so testosterone secretion remains unaffected.
