Infectious causes are often due to unprotected sex, transmitting Candida, trichomonas, chlamydia, mycoplasma, gonococcal bacteria, or other bacteria. Non-infectious factors are mostly due to phimosis, insufficient hygiene, and the accumulation of unclean substances between the foreskin and glans, known as smegma, which can irritate the local foreskin and mucosa, leading to inflammation and causing glans itching. When the foreskin is long and the foreskin opening is tight, if it is forcibly retracted and not promptly, the tight foreskin opening acts like a tight ring, tightly constricting the glans of the penis. This is known as phimosis. Below the phimosis, blood circulation and lymphatic drainage are obstructed, causing swelling of the penis and foreskin, making repositioning even more difficult.
