How is it best to treat epididymitis?

Patient's question:

Recently, I've been feeling a lot of pain in my testicles, and this week alone, I've had a fever twice. Usually, I feel cold, and when I take a shower, I touch my testicles and feel like there's a lot of liquid inside. Please ask: How is it best to treat epididymitis?

Doctor's answer:

(1) General treatment for bed rest involves using a scrotal support or a specially designed soft pad to hold the scrotum, which can alleviate symptoms. For severe pain, painkillers can be used. Local heat therapy can relieve symptoms and promote inflammation reduction. However, early use of heat therapy may relieve pain but also carries the risk of spreading infection. Therefore, early use of ice packs for local cold compressions is recommended. Sexual activity and physical labor can exacerbate infection, so they should be avoided.
(2) Antibiotics should be selected based on the sensitivity of the bacteria. Typically, intravenous administration is used for 1–2 weeks, followed by oral antibiotics for 2–4 weeks to prevent the condition from becoming chronic inflammation.
(3) If antibiotic treatment is ineffective and testicular edema is suspected, orchidectomy with epididymal drainage should be performed. The epididymal tunica vaginalis can be incised longitudinally or transversely, but care must be taken to avoid damaging the epididymal ducts.
2. Treatment for chronic epididymitis. The effectiveness of medication alone in treating chronic epididymitis is inconsistent.
