In acute cases, the volume may decrease by half within a few hours, and white blood cell counts can rise to 20,000–30,000 per cubic millimeter. It is often confused with testicular torsion. If gently supporting the scrotum alleviates the pain, it indicates orchitis, whereas testicular torsion causes the pain to intensify when the scrotum is lifted. In chronic orchitis, the vas deferens becomes thickened, and the spermatic cord may also thicken. The pain is typically dull, and a careful local examination reveals a hard mass in the epididymis with mild tenderness.
