How to treat varicocele and what are the treatment options?

Patient's question:

When I was taking a shower, I noticed that one of my testicles is much larger than the other, and it seems to be lower on one side. When I touched it, I felt that the veins were very prominent, almost like earthworms coiled inside the testicle. Please ask: If I have varicocele, what treatment options are available?

Doctor's answer:

Primary varicocele is generally asymptomatic, and surgical treatment is usually not required for individuals seeking fertility, as elastic stockings or scrotal supports can be used. Men with severe symptoms or infertility due to varicocele accompanied by abnormal sperm should undergo surgical treatment. Asymptomatic mild varicocele does not require treatment.
Non-surgical treatment: For mild varicocele or those accompanied by neurasthenia, scrotal compression and cold compresses may be recommended.
Surgical treatment: For severe varicocele, persistent sperm count below 20 million per cubic centimeter for three consecutive times, or testicular atrophy; if the varicocele disappears when lying flat, high ligation of the internal spermatic vein can be performed.
