10 weeks no fetal pole and heartbeat

Patient's question:

Age 29, no history of miscarriage, last menstrual period on December 26, 2012. Had first intercourse on January 13, 2013. Urine test positive on February 7. Ultrasound performed on February 23 showed a gestational sac measuring 22mm long, 22mm wide, and 9mm thick, with a yolk sac visible, but no embryonic pole or fetal heartbeat. Progesterone level was 20.7 nmol/L.
The doctor prescribed progesterone tablets to be taken twice a day. A follow-up ultrasound on March 5 (10 days later) showed a gestational sac measuring 23mm long, 26mm wide, and 11mm thick, still without an embryonic pole or fetal heartbeat. Progesterone level was 24 nmol/L, HCG 35,317.39. Progesterone was still low, and the gestational sac showed minimal growth. No abdominal pain or bleeding. The doctor said the chances were slim, suggesting a missed miscarriage.
I’m reaching out to those with experience for help. This is my first pregnancy, and I don’t want to give up. I’m in so much pain and heartache. I sincerely thank you in advance.
Menstruation stopped for 9 weeks. If the actual pregnancy period is calculated from January 15 or 20 to now, it has been 7 weeks.

Doctor's answer:

In your case, it is possible that the fetus has stopped developing, and the fetus has died inside the uterine cavity. Common causes of fetal death include umbilical cord abnormalities, fetal malformations, and maternal diseases that lead to placental dysfunction, resulting in insufficient oxygen supply and fetal hypoxia leading to death. If the dead fetus remains in the uterus for too long, it can cause maternal coagulation dysfunction, leading to uncontrollable postpartum hemorrhage during delivery, which poses a significant risk to the mother. Therefore, timely diagnosis and treatment are essential. It is recommended to induce abortion as soon as possible to avoid causing maternal DIC.
