The ultrasound examination revealed a chocolate cyst of the ovary measuring 41x31mm, with the possibility of adenomyosis. The MRI showed the cyst measuring 41x41x32mm. The rectal ultrasound endoscopy detected a low-echo area with a cross-sectional size of 13.9x6.3mm, extending from the outer to the inner layers, involving the serosa, the propria muscularis, the submucosa, the muscularis mucosae, and the mucosa, which remained smooth and continuous.
Years ago, one of her ovaries was removed due to a mucinous cystadenoma.
The doctor diagnosed pelvic endometriosis, ovarian endometriosis, rectal endometriosis lesions, vaginal rectal septum endometriosis, and adenomyosis of the uterus.

1. Mucinous cystadenoma is a benign tumor.
2. Do not aim to have children with the intention of treating endometriosis (EMS), as it is already quite difficult to conceive with this condition and requires significant financial investment. You may have had children when EMS was not yet diagnosed or was not severe. EMS is a progressive and worsening disease.
3. Research has confirmed that repeat surgery offers no advantage in preventing recurrence of EMS and carries the risk of iatrogenic seeding. Therefore, repeat surgery is not recommended.
4. You did not mention your age.
5. It is recommended to take oral short-term contraceptives long-term to suppress ovulation and reduce recurrence. After taking them for half a year, stop the medication until menstrual pain or other discomfort symptoms return, then restart the medication for another half year to allow the ectopic endometrium to atrophy.
