2. Abnormal cervical canal position: Chronic pelvic inflammation or endometriosis may cause the uterus to be extremely retroverted, retroflexed, or antiflexed, making the external cervical os adhere to the anterior fornix. This results in the shallowing of the posterior fornix, losing its function as a sperm reservoir, thereby hindering sperm ascent. Additionally, cervical elongation, shortening, or prolapse can also obstruct sperm ascent.
