Pregnant for nine months got candidiasis of the vagina using Clotrimazole

Patient's question:

Pregnant for nine months with candidal vulvovaginitis, can I use clotrimazole cream?

Doctor's answer:

Hello, don't worry too much, it's not a big deal. It's fine to use externally during the late stages of pregnancy. Clinically, it's relatively common for pregnant women to develop candidiasis (commonly known as moldy vaginitis). This is because the high estrogen levels during pregnancy create a vaginal environment with high glycogen content, providing rich conditions for the growth of Candida albicans (the fungus commonly referred to as mold). Therefore, pregnant women are more susceptible to candidiasis. During pregnancy, Candida reproduces rapidly, and the condition is exacerbated by the fact that high estrogen levels can directly stimulate the growth of Candida. This is why candidiasis often occurs during pregnancy and tends to recur despite treatment.
Clinically, common medications for treating candidiasis include: nystatin tablets, micafungin effervescent vaginal tablets or suppositories, clotrimazole tablets (Kanindin tablets), and miconazole nitrate suppositories (Daktarin suppositories). These can all be inserted vaginally under the guidance of a physician. External creams such as clotrimazole cream and Daktarin cream can be applied to the external genital area daily. Additionally, a 2%–4% sodium bicarbonate (baking soda) solution can be used to rinse the vagina or external genital area, or traditional Chinese medicine (TCM) or TCM preparations can be used to clean the external genital area.
During pregnancy, especially in the first three months, pregnant women should avoid or use oral antifungal medications with caution.
Prevention:
- Keep the external genital area dry.
- Maintain good hygiene.
- Wear loose, highly breathable underwear.
- If there is anal or intestinal Candida infection, appropriate treatment should be administered.
- Sexual transmission is also possible. If one partner is infected, the other should be treated simultaneously. If the condition does not improve after treatment, further examinations may be necessary.
