I don't seem to have nipples, only areolas, and there's a small hole in the middle of the areola. I've already had children, but I've never had nipples to breastfeed. It's normal. The small hole on your nipple is a milk duct. Since you haven't let your child nurse from you, your nipple appears smaller. If you had let your child nurse, it wouldn't be like this, and your nipple might have become longer. Therefore, you don't need surgery.
