Irregular menstruation, excessive leukorrhea - what to do?

Patient's question:

After 20 days, my period came. The amount was not much, and it lasted for three days. Later, on the 28th day, it came again, and the amount was slightly more than before, but it took nine days to clean up. This month, on the 20th day, it came again, and there was some yellow discharge. I have had an IUD for a year and a half, and it was always normal before.

Doctor's answer:

1. Premature menstruation. Menstrual irregularity refers to a sudden shortening of the menstrual cycle, shorter than 21 days, with at least two consecutive cycles, while the menstrual flow remains normal. It is characterized by a biphasic basal body temperature in ovulatory dysfunctional uterine bleeding, with a short follicular phase of only 7–8 days, or a luteal phase shorter than 10 days, or insufficient temperature rise of less than 0.5°C.
2. Delayed menstruation. Regular menstrual cycles with a delay of more than 7 days, or even 40–50 days between cycles, occurring consecutively for at least two cycles, while the menstrual flow remains normal. In ovulatory cycles, there is a biphasic basal body temperature, but the follicular phase is prolonged with a lower high-temperature phase. In anovulatory cycles, there is a monophasic basal body temperature.
3. Extended menstruation. Normal menstrual cycles with normal flow, but the duration of menstruation exceeds 7 days, or even lasts for two weeks. In cases of inflammation, there is mild lower abdominal pain that worsens during menstruation, along with increased yellow or yellowish-white, viscous, and odorous leukorrhea. In cases of incomplete luteal regression, there is excessive menstrual flow. In cases of prolonged endometrial repair, there is persistent vaginal bleeding after a normal menstrual period.
4. Menstrual disorders. Irregular menstruation with unpredictable timing, either premature or delayed, with cycles shorter than 21 days or longer than 35 days.
5. Mid-cycle bleeding (also known as intermenstrual bleeding or ovulatory bleeding). This refers to bleeding occurring between two regular menstrual cycles due to a temporary decline in estrogen levels, causing the endometrium to lose hormonal support and shed.
