Patient's question:
Female, under 50 years old, good physical health, menstrual pain, unbearable pain, sweating profusely, palpitationsDoctor's answer:
The treatment options for this condition are numerous, and clinical decisions should be individualized based on the patient's age, symptoms, and fertility requirements. Surgical and pharmacological treatment plans can be chosen simultaneously.1. Pharmacological Treatment
(1) Symptomatic Treatment For patients with mild symptoms and only a need for relief from dysmenorrhea, nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen, indomethacin, or naproxen can be used for symptomatic management during menstrual periods.
(2) Pseudopregnancy Therapy For patients with mild symptoms and no current desire for fertility or those nearing menopause, oral contraceptives or progesterone can induce decidualization and atrophy of ectopic endometrium, thereby controlling the progression of adenomyosis.
(3) Intrauterine Device (IUD) For patients with heavy menstrual bleeding, dysmenorrhea, and no current desire for fertility, an IUD containing high-efficiency progesterone can be chosen. It releases progesterone locally in the uterus to control the development of ectopic lesions and should be removed or replaced after five years.
