What is the reason for persistent abdominal pain after an induced abortion?

Patient's question:

What is the reason for persistent abdominal pain after an abortion?

Doctor's answer:

Generally, on the day of the surgery, there may be mild lower abdominal discomfort, which will disappear afterward or occasionally recur. If the patient experiences significant postoperative abdominal pain, it is considered abnormal and may be caused by the following reasons:
(1) Incomplete abortion: Sometimes, an abortion procedure may fail. Incomplete abortion refers to the situation where the surgical procedure does not completely remove all the villi and decidua tissue from the uterine cavity. After the surgery, the uterus contracts intermittently to expel the remaining embryonic tissue. During this time, the patient may experience intermittent abdominal pain, which is severe during attacks, accompanied by increased vaginal bleeding, bright red in color, and the presence of large blood clots. The pain may subside after the blood clots are expelled, and this cycle may repeat.
In cases of incomplete abortion, a uterine curettage is usually performed to remove the remaining embryonic tissue from the uterine cavity, which will naturally alleviate the abdominal pain. Postoperatively, enhanced anti-inflammatory treatment should be administered to prevent infection.
