Patient's question:
Should a hole be left after wound recovery from Bartholin gland cyst surgery? During the surgery, a rhomboid incision was made. Now, 5 days post-surgery, the stitches are still in place, and there is white necrotic tissue around the wound. I use Compound Huangteng Wash once a day to clean it. I want to ask, will my wound close? If it closes, will it affect the drainage of Bartholin gland fluid?Doctor's answer:
【Treatment Measures】Since cysts can lead to infection, surgical treatment should be pursued. In the past, cyst excision was commonly performed, but it often involved bleeding. If the cyst wall extends near the urethra, the surgical procedure may be difficult, or the cyst wall may not be completely removed, leading to a risk of recurrence. Severe scarring can cause difficulties in sexual intercourse, so now cyst excision is only used for suspected malignant lesions. Cystoplasty (pouch suture) has been proven through years of practice to be a simple, safe method with few complications and a low recurrence rate, while preserving glandular function. It can also be used for Bartholin gland abscesses. Ostomy is suitable for larger and repeatedly acute recurrent cysts; excision is suitable for infected cysts. For larger cysts, a CO2 laser (power 25W) can be focused at the lowest point to incise the skin by 0.5 cm, or a D∶YAG laser fiber (power 20W) can be used to incise the lowest point of the mass by 0.5 cm. Drain the cyst contents, and perform daily cleaning and dressing changes on the drainage opening. After surgery, a small opening will form on the inner side of the labia, and if the cyst recurs, pus will automatically flow out from the created opening.
