What are the precautions to be taken when examining semen and sperm?

Patient's question:

What are the precautions to be noted when checking semen and sperm? I and my wife have had sexual intercourse many times, but she hasn't shown any signs of pregnancy yet. Could this be related to my sperm? I want to check if there are any problems with my sperm. What are the precautions to be noted when checking semen and sperm?

Doctor's answer:

Hello, please note the following:
1. Sperm collection must be performed only when in good health.
2. Abstain from sexual activity for 3-5 days before collection. A period that is too short (e.g., 2 days) may result in reduced sperm density and semen volume, while a period that is too long (e.g., >7 days) may decrease sperm motility. During abstinence, it is also recommended to avoid smoking, alcohol, and medications that may affect sperm production, such as sex hormones.
3. The best method for collecting semen is masturbation. Intercourse-based ejaculation may lead to the loss of highly motile sperm and may also contaminate the sample with vaginal secretions, reducing sperm motility.
4. Before collection, wash your hands and genital area, especially the glans penis, with warm water. The environment should be quiet and free from distractions. Early morning collection is ideal, and it is best to do so at a specialized hospital.
5. The collection container should be sterile, clean, and dry. The temperature of the container should be the same as the room temperature. The opening of the container should not be too small to prevent semen from spilling out. The container should not be excessively large. Avoid using condoms for collection, as the latex in condoms and lubricants or talcum powder on them may affect sperm motility. Additionally, semen sticking to the condom may affect the measured volume of semen.
6. After collection, store the semen container at approximately 37°C. Avoid tilting the container after collection and send it for testing immediately. Aim to have the test completed within a short period, generally no more than 2 hours. Prolonged storage or extreme temperatures (too high or too low) will both affect sperm motility.
