Sperm motility is divided into 4 grades:
Grade a indicates excellent sperm motility, with rapid straight forward movement;
Grade b indicates good sperm motility, with straight forward movement;
Grade c indicates average sperm motility, with only forward curved movement;
Grade d indicates poor sperm motility, with only localized wriggling.
Sperm with rapid forward movement (Grade a) and slightly slower forward movement (Grade b) (Grade a + Grade b) < 50% or Grade a sperm < 25%.
① Mild asthenozoospermia refers to Grade a + Grade b sperm < 50%, but > 30%, or Grade a sperm < 25%, but > 10%.
② Moderate asthenozoospermia refers to Grade a + Grade b sperm < 50%, but > 30%, with Grade a sperm < 10%.
③ Severe asthenozoospermia refers to the number of Grade b + Grade c sperm < 30%, with no Grade a sperm.
In asthenozoospermia, sperm lack the strength to move forward, making it difficult for them to pass through the cervical canal, uterine cavity, and reach the fallopian tubes to combine with eggs, thus reducing fertility.
Common causes of asthenozoospermia include:
1. Incomplete maturation or damage to the spermatogenic epithelium in the testes, leading to poor sperm quality and weak motility;
2. Low seminal fluid volume;
3. Variations in seminal plasma, such as inflammation in the epididymis, seminal vesicles, or prostate, which can negatively affect sperm activity and survival due to changes in pH, oxygen supply, nutrition, and metabolism. If antisperm antibodies are present, they can cause sperm agglutination, rendering them inactive.
