How should one treat severe oligospermia?

Patient's question:

Hello doctor, I would like to ask what methods should be used to treat sperm agglutination? Because I have been trying to conceive for half a year without success, I went to the hospital for a semen analysis and found that there were many dead sperm in my semen. The doctor said I have sperm agglutination, which affects fertility. How should sperm agglutination be treated?

Doctor's answer:

Hello, azoospermia refers to a condition where multiple semen analyses show no abnormal sperm morphology and density is within the normal range, but the majority of sperm are dead. Under normal circumstances, more than 70% of viable sperm should remain in the semen within 1 hour of ejaculation. If dead sperm exceed 40%, it can affect fertility. An increase in dead sperm caused by improper examination methods or failure to follow normal procedures does not fall under this condition. Clinically common causes of dead sperm include prolonged abstinence, excessively high or low environmental temperatures for sperm, varicocele of the seminal vesicles, inflammation and infection of the reproductive tract, immune suppression, endocrine disorders, and smoking and alcohol intoxication, all of which can inhibit sperm production. It is recommended that you undergo a thorough etiological examination and actively treat the underlying cause. Wishing you good health!
