Patient's question:
She has been using the IUD for 5 years and had it removed yesterday, December 18, 2005. There was a small amount of bleeding. Please advise on how to treat it. Thank you!Doctor's answer:
Post-IUD Insertion PrecautionsIn the first month after insertion, some women may experience breakthrough bleeding or spotting, which may recur in the following months. Some women may also have prolonged periods or excessive bleeding, especially in the first 1–3 months after insertion. These are normal phenomena and do not require special treatment; they will gradually improve over time. If bleeding persists for an extended period, appropriate antibiotics may be taken to prevent infection, and medical attention should be sought if necessary.
Rest for 2 days after IUD insertion and 1 day after IUD removal. Within the first week after insertion, avoid excessive physical labor, particularly squatting, to prevent heavier bleeding or IUD displacement.
If some women have already entered menopause or have irregular periods or have become postmenopausal, the IUD should be removed. It is best to have the IUD removed within one year after menopause. If removal occurs more than one year after menopause, due to declining ovarian function, low estrogen levels, vaginal mucosal atrophy, and uterine shrinkage, the procedure may become more difficult and risky.
After IUD insertion, have a pelvic exam and IUD check-up one month after the first menstrual period and again three months later. Subsequently, have an annual IUD check-up and a gynecological examination to determine if the IUD has fallen out.
Within three months after IUD removal, pay attention to checking for IUD displacement during periods of heavy menstrual bleeding or constipation. Maintain regular bowel movements.
After IUD removal, maintain vaginal cleanliness and hygiene. Wash the genital area with warm water daily and avoid getting cold. Refrain from sexual activity for two weeks, as well as from sitz baths or tub baths, to prevent infection and inflammation, which may hinder recovery. This is also one of the important precautions after IUD removal.
Precautions
Pay attention to nutrition and consume more iron-rich foods to prevent anemia caused by excessive menstrual bleeding.
