How to treat bleeding in the middle of the menstrual cycle?

Patient's question:

Hello Doctor! I am 28 years old. Every half month during the middle of my menstrual cycle, I have bleeding. Sometimes it is heavy with dark color, and sometimes it is less like watery blood, but still significantly less than a normal menstrual flow. It sometimes lasts for three days, and at other times, it intermittently continues for seven or eight days. This has been going on for six or seven years. How should I be treated? I have taken medication before, but it had no effect. Also, should I avoid swimming or sexual activity during this period? Thank you, Doctor!
First follow-up question: Thank you, Doctor! Why does the bleeding come and go, and why does it last for seven or eight days without stopping? It is very frustrating. Also, the color of the blood sometimes changes from dark to light, and sometimes it is brown.

Doctor's answer:

Hello, it is possible that it is ovulation bleeding.
During the mid-cycle, which is the ovulation period, due to a temporary decline in estrogen levels, the endometrium loses hormonal support, leading to partial shedding of the endometrium and causing regular vaginal bleeding, known as ovulation bleeding. In traditional Chinese medicine, it is referred to as "intermenstrual bleeding."
Treatment
1. Western medicine treatment: Diethylstilbestrol 0.25–0.5 mg per day, starting 3 days before ovulation and stopping 2–3 days after the bleeding stops.
2. Traditional Chinese medicine treatment: Liuwei Dihuang Wan, Gujing Wan, and Xuefu Zhuyu Wan.
Prevention and Care
1. Pay attention to menstrual health, maintain cleanliness of the external genitalia, and prevent pelvic infection.
2. Keep a calm mood and avoid excessive stress.
You can swim and have sexual intercourse.
