What is the reason for continuous bleeding after the hymen is torn?

Patient's question:

Doctor, I am 22 years old. Yesterday, I had sexual intercourse with my boyfriend (I was a virgin before this), and later, my vagina started bleeding. At first, I thought it was my hymen breaking, but it kept flowing non-stop all night, and there were also large clots coming out. I want to know what is going on, and I hope you can reply to me as soon as possible. I am very worried! Thank you!

Doctor's answer:

Your situation is likely due to the onset of your menstrual cycle. It is recommended to continue monitoring. During the first sexual intercourse, the hymen is torn, resulting in a laceration and bleeding. Due to the varied shapes of the hymen, the extent of the rupture can differ significantly. A septate hymen with two openings tends to cause more bleeding and relatively intense pain. A labial hymen, on the other hand, results in minimal bleeding with almost no pain. A thick and highly elastic bell-shaped hymen may not rupture at all.
When the hymen tears, it does not gush blood like a major blood vessel would; the bleeding is usually minimal, with only a few drops or dozens of drops. In some women, there may not be any bleeding at all.
Therefore, the absence of bleeding cannot confirm that a person is still a virgin.
