First-time sexual intercourse without bleeding, feeling loose, what's going on?

Patient's question:

I had my first sexual experience with my fiancé, and it was also the first time in my life. I don't know why I didn't bleed, and he felt it was quite loose. What's going on?

Doctor's answer:

First sexual intercourse does not necessarily involve bleeding. It can range from a few milliliters or drops to heavy bleeding. Even a broken hymen may not result in bleeding. It is not rare for a virgin to not bleed during her first sexual intercourse, and there are several reasons for this, such as a thick and tough hymen, a large hymen opening with fewer blood vessels, or congenital absence. The hymen may also break during vigorous activity or the use of tampons—perhaps it had already torn before you had sexual intercourse. Some hymens may even break without bleeding. Additionally, in some women, the hymen can be very close to the edge of the vaginal wall, so it may not break during intercourse at all. Some may bleed the first time their hymen breaks, and they may still bleed during subsequent sexual activity. Therefore, there can be no bleeding during first sexual intercourse.
