What should I do if there is no gestational sac after a 39-day ultrasound examination?

Patient's question:

Medical History Description: 40 days of amenorrhea. On day 39, a transvaginal ultrasound was performed, showing no abnormalities in the uterine adnexa, a 12mm endometrial thickness, and an 87mm anechoic area in the right adnexal region. It was recommended to follow up in 5 days. On day 40, there was a small amount of bleeding, and a pregnancy test showed a faint positive result. Is it delayed menstruation or ectopic pregnancy? There is a slight pain in the left lower abdomen. Should I go to the hospital for further examination now?
Past Treatment and Effect: I previously had endometriosis and a left ovarian cyst. In the past six months, physical examinations and this ultrasound did not detect an ovarian cyst.
Help Needed: Should I go to the hospital for an examination immediately? The doctor originally said to follow up in 5 days.

Doctor's answer:

Consider that it is not an ectopic pregnancy, you have experienced spotting, and it is suspected to be threatened miscarriage. After taking medication, you had small blood clots, which are considered to be medication-induced miscarriage. If you want to miscarry, you can go to work as long as the vaginal bleeding is not heavy. After the miscarriage, you need to take oral anti-inflammatory and uterine contraction medication for 3-5 days, rest for 2 weeks, and avoid sexual activity and tub baths for one month. Generally, you should be able to see an intrauterine gestational sac around 40 days of pregnancy. If hCG is greater than 620 or progesterone is greater than 20, it means it will not be an ectopic pregnancy. If it is an ectopic pregnancy, oral miscarriage medication cannot be used.
