Every time after ejaculation, it flows out and does not get pregnant

Patient's question:

Hello, I have multiple sexual encounters with my girlfriend, and after ejaculation, there is discharge from her vagina. What could be the reason? Is it my problem or her problem? You said it's a normal phenomenon, but when we had our first sexual encounter, she didn't bleed. Then every time after ejaculation, it flows out, and we also haven't gotten pregnant.

Doctor's answer:

After coitus, semen flowing out of the vagina is a common phenomenon. In fact, although each ejaculation can expel about 2-5 milliliters of semen containing billions of sperm, only a very small number of sperm can pass through the cervix and uterine cavity to reach the fallopian tubes to await fertilization. Generally, only 15-50 sperm can do so, and rarely more than 200. Among these, only one high-quality sperm combines with the egg. The rest of the sperm are expelled from the vagina along with the semen.
To prevent semen leakage and increase the chance of conception, lying supine for two hours after intercourse ensures that the semen covers the cervix, reducing its leakage. As for the expulsion of excess semen from the vagina after intercourse, this is a natural phenomenon and generally has no impact on fertility.
