How is ascites considered severe after transplantation?

Patient's question:

I got married last year. Recently, I've been experiencing lower abdominal pain, especially during my period. I'm very worried about it affecting my fertility, and I'm also extremely sensitive to cold and coldness. Could you please tell me how to determine if ascites is severe after transplantation?

Doctor's answer:

The main cause of ascites after IVF transfer is primarily due to the development of multiple follicles, excessively high estrogen levels, which induce ovarian hyperstimulation. During the ovulation induction phase, some patients may develop a significant number of follicles (typically younger patients or those with better ovarian function). On one hand, this provides more resources for the success of the IVF procedure, but on the other hand, it also increases the risk of ascites.
Experts from the Dongguan Reproductive Medicine Center state that from a symptomatic perspective, ascites is characterized by the accumulation of a large amount of fluid in the abdomen, causing the patient's abdomen to swell, sometimes resembling pregnancy for several months, accompanied by abdominal pain. Generally, patients can alleviate symptoms by consuming more chicken egg whites (cooked eggs), eating diuretic foods (such as watermelon juice, winter melon soup, and drinking plenty of water), and urinating frequently. However, if this does not help, medical attention is necessary. Severe ascites requires attention: if it occurs before the transfer, it is recommended to postpone the IVF procedure until the ascites resolves before scheduling the transfer. If severe ascites appears after embryo transfer, it may affect this pregnancy. Mild cases typically require medication for regulation, while severe ascites necessitates hospitalization for observation and adherence to medical advice.
Ovulation induction aims to "recruit" more follicles in the ovaries, so it is essential to rest, stop exercise, and fitness activities. Additionally, ovulation induction drugs must be stored according to sleep instructions. If mild ascites occurs, it is advisable to wait and see. Foods such as crucian carp soup, winter melon pork rib soup, soy milk, plain water, watermelon, grapefruit, and cherries should primarily focus on high-protein foods and diuretic foods, with moderate walking.
