Why is there abdominal pain when menstruation is delayed by four days?

Patient's question:

10 days since my last menstrual period, but it hasn't arrived this month. The past two days, I've suddenly experienced sharp lower abdominal pain, and my stomach feels bloated. My breasts are also swollen! When I lie down, pressing slightly above my right lower abdomen causes pain. Additionally, my inner thighs feel sore, as if they're being pinched uncomfortably!
To provide some context, I haven't had any sexual activity recently, and I had two ultrasound exams at the end of June and the end of July. Everything was normal.
What help do I need: Could you please explain the possible reasons for my abdominal pain? What should I do? Also, could you explain the causes of the other two issues?

Doctor's answer:

The causes of menstrual disorders include organic lesions or medication, as well as neuroendocrine dysfunction. Generally, when women suffer from uterine appendage diseases, endocrine disorders, endometrial atrophy caused by induced abortion, oral contraceptives, or conditions near menopause, they may experience menstrual disorders. Based on your symptoms, it is mainly considered to be due to endocrine disorders. It is recommended that you visit a hospital for a sex hormone test to check your hormone levels and receive symptomatic treatment. A delay or advancement of 5 days is normal. If there are no obvious symptoms, no special treatment is needed. If symptoms are present, appropriate sedatives or analgesics can be given.
