The main manifestations of miscarriage are vaginal bleeding and abdominal pain. In early miscarriage, vaginal bleeding often occurs first, followed by lower abdominal pain. If the miscarriage occurs before the 8th week of pregnancy, the villi have not yet matured and their connection with the uterine decidua is not firm. As a result, the entire blastocyst and villi are often completely detached from the uterine wall and expelled. During expulsion, the embryo is wrapped in decidua and blood clots, and bleeding is usually minimal. If the miscarriage occurs between the 8th and 12th weeks of pregnancy, the villi have developed vigorously and deeply embedded in the decidua, forming a firmer connection. In this case, the placenta often cannot be completely detached and expelled along with the fetus. Therefore, it is common to see only the fetus expelled alone, or the fetus along with part of the placenta. This leaves some tissue remaining in the uterine cavity, which hinders uterine contraction and affects the closure of blood sinuses, leading to more significant bleeding. After the 12th week of pregnancy, the placenta is fully formed. During miscarriage, the placenta is expelled intact after the fetus is delivered. Therefore, the process of late miscarriage is similar to that of preterm labor or full-term delivery, starting with intermittent uterine contractions, followed by cervical dilation, and then the delivery of the fetus and placenta. As a result, late miscarriage often begins with abdominal pain, followed by vaginal bleeding, which is usually minimal.
