Hello, the maintenance of a normal menstrual cycle depends on the coordination of the hypothalamus-pituitary-ovary function and the cyclical response of the endometrium to sex hormones. Any problem in the above links may lead to amenorrhea. Most amenorrheic patients do not ovulate, hence they cannot conceive.
Except for physiological amenorrhea (seen in prepubertal girls, postmenopausal women, and pregnant women), other types of amenorrhea are pathological, and their causes are as follows:
(1) Uterine amenorrhea: Caused by the endometrium not responding to hormones or responding poorly. It is commonly seen in conditions such as endometrial tuberculosis, endometrial damage (e.g., uterine atrophy after curettage), endometrial adhesions, and endometrial hypoplasia. In such cases, even with artificial hormone regulation (artificial cycle), the endometrium will not shed and bleed.
(2) Ovarian amenorrhea: Primarily caused by ovarian diseases. Due to excessively low estrogen levels, the endometrium cannot grow, such as in congenital ovarian hypoplasia, premature ovarian failure, and unresponsive ovaries. In these individuals, estrogen levels are low while gonadotropin levels are normal or elevated, and artificial cycle treatment can be applied.
(3) Pituitary amenorrhea: Amenorrhea is related to pituitary dysfunction and can be seen in cases of cranial trauma, Sheehan's syndrome, or after cranial radiation therapy.
(4) Hypothalamic amenorrhea: The issue lies in the hypothalamus, which can be caused by organic neurological diseases such as inflammation, tumors, or ischemia, or by psychological factors, environmental changes, systemic diseases, malnutrition, and drug effects.
Amenorrhea refers to the condition in women of reproductive age where menstruation does not occur for more than 3 months consecutively.
