What are the reasons for female infertility?

Patient's question:

2008 Right adnexa surgery performed, can you take a look at this ultrasound report:
2012.12.17 Anterior uterus, size 66.78 cm, endometrium 1.0 cm, homogeneous echo, muscle layer echo slightly inhomogeneous. Cervix shows multiple cystic echoes, the largest about 2.11.0. Right ovary shows an approximately 1.51.0 cm follicle, left ovary shows a 2.62.4 cm follicle. Liquid area between uterus and bladder, size approximately 6.42.92.2 cm.
Diagnosis: Enlarged uterus, considered adenomyosis, multiple cervical glandular cysts, cystic encapsulated effusion between uterus and bladder.
This was done on the first day of the menstrual period. I have been monitoring the ovulation (follicle) of the previous month, but ovulation did not occur. The menstrual period has now arrived. Will this follicle still ovulate?
Last month, I took four tablets of clomiphene and ovulated, but did not get pregnant. This month, I haven’t taken it, and I don’t know what to do.

Doctor's answer:

For adenomyosis, there are currently conservative treatments, medication adjustments, Mirena IUDs, etc. Different treatment plans are adopted based on different conditions. The Mirena IUD is a medication-releasing ring that can alleviate symptoms and control the progression of the disease. It is currently one of the most effective treatments for adenomyosis, endometriosis, and other conditions. Ultrasound-guided fine-needle aspiration can also be used for treatment.
