How can candidiasis be treated after a recurrence?

Patient's question:

Detailed medical condition and consultation purpose: Two weeks ago, I got checked and was diagnosed with candidal vulvovaginitis. The doctor prescribed medication for a week, including a wash, an insert, and clotrimazole cream. After completing the treatment, I felt itchy again. I went back for a follow-up visit. The doctor asked when my period would start. I said it would be another ten days. The doctor then said, "You don't need to take medication now. Continue washing, inserting, and using the cream after your period." For these ten days, if you feel itchy, just apply the cream. If not, you don't need to use any medication. However, yesterday my period started, and I began feeling itchy again down there, along with a fishy odor. There are also many grain-like spots on top. I don't know if these spots are caused by the fungus. During my period

Doctor's answer:

Vaginal candidiasis is recommended to first use an alkaline solution such as 2-4% sodium bicarbonate or soapy water to rinse the external genitalia and vagina, altering the vaginal pH. Then, insert Nystatin vaginal suppositories deep into the vagina, twice a day or once a night. If it is a recurrence, it may be caused by a carrier in the digestive tract. In this case, you can add oral Nystatin 500,000 U, four times a day, and apply 2% gentian violet locally.
In daily diet, it is advisable not to eat foods high in sugar or spicy and irritating foods. You can consume more yogurt. Personal hygiene should be maintained, and underwear should be changed frequently. Avoid wearing pantyhose. Used underwear, basins, towels, etc., should be boiled and dried in the sun. During treatment, sexual activity should be avoided, and the male partner is also advised to apply medication locally.
