Is it possible to get pregnant after a missed period?

Patient's question:

I came in on November 8th, and my period ended on the 10th. On the morning of the 15th and 16th, we had sex. On the afternoon of the 20th, we had sex again. My menstrual cycle is 27 days. Today's ultrasound shows that the uterus is smaller, with a clear outline, uniform echo of the uterine wall, and a gestational sac measuring 2.31.8cm inside the uterine cavity. There is no fetal pole, no fetal heartbeat, or fetal movement visible inside the sac. The intrauterine pregnancy is approximately 40 days. Can this child be kept? Can you confirm which day I will give birth? I desperately need your help.

Doctor's answer:

You can only speculate about the inability to conceive based on a single ultrasound. If you had two ultrasounds, you could compare them side by side. However, with only one ultrasound, based on your last sexual intercourse on the 20th, it would be 26 days by the time of the thorough examination, which is 40 days after conception. Under normal circumstances, a gestational sac should be visible at this stage. However, since you still cannot see a gestational sac, it could be a sign of poor embryonic development. But this is not definitive; blood tests are also needed to make a more accurate assessment.
