Doctor's answer:

Hello, adenomyosis is a benign gynecological condition where the endometrium grows into the uterine muscle. The cause is unclear, and it can lead to symptoms such as dysmenorrhea (painful menstruation), dyspareunia (pain during intercourse), infertility, and miscarriage. Clinically, it is primarily characterized by dysmenorrhea, increased menstrual flow, and uterine enlargement. However, its etiology, age of onset, clinical manifestations, and treatment principles differ from those of extrinsic endometriosis. It is more common in postmenopausal women aged 40–50. The reason is that the basal layer of the endometrium directly extends into the muscular layer, rather than through implantation or metaplasia.
Recommendation: Currently, medication can be used for treatment. However, clinical experience over the years has shown that medication for adenomyosis is ineffective and does not provide a cure, nor does it eliminate dysmenorrhea.
Traditional Treatment: The definitive cure is a total hysterectomy. After lesion removal surgery, if about 1/4 of the uterine wall is preserved, the uterus can return to normal size within two years. In addition to significantly reducing or eliminating dysmenorrhea, fertility may also be preserved.
Therefore, it is recommended to treat the condition first before considering pregnancy. Wishing you good health!
