Patient's question:
Detailed medical condition and consultation purpose: Menstrual pain for many years, each time it's extremely painful, accompanied by diarrhea and a feeling of nausea. After giving birth last year, my first period after weaning was completely painless, with a relatively heavy flow. However, the next period caused pain, though not as severe as before. Is there any way to relieve the pain? Each period is quite light, almost non-existent—could this be due to hypomenorrhea? Because the first period was painless and the flow was heavy. Please help the experts advise.
My menstrual cycle is regular, approximately 28 days.

Doctor's answer:
Hello: According to your condition, you should go to the hospital for an endocrine examination. Dysmenorrhea is defined as lower abdominal pain during menstruation or in the days before or after menstruation, which affects work or health. Dysmenorrhea can be classified into primary and secondary types. For secondary dysmenorrhea, etiological treatment should be administered. For primary dysmenorrhea, when the pain is severe, "atropine" 0.5 mg can be given intramuscularly, or symptomatic treatment can be provided using pain relievers such as "aspirin." Progestogen therapy can also be used, such as taking "norethisterone" 2.5–5 mg or "medroxyprogesterone" 4–8 mg orally once a day, starting on the 5th day of the menstrual cycle, and continuing for 22 days, with a 3-cycle course. Additionally, "progesterone" 10–20 mg can be given intramuscularly once a day, starting 7 days before the onset of menstruation, and administered for 5 days. Estrogen therapy, such as taking "diethylstilbestrol" 0.5–1 g orally once a day, starting on the 6th day of the menstrual cycle, and continuing for 20 days, can also be chosen.
