Doctor's answer:

Epididymitis refers to an infectious disease of the epididymis, commonly occurring in adolescents. It can be unilateral or bilateral. Pure epididymitis is rare and often coexists with orchitis. It can be caused by non-specific infections of general bacteria or specific pathogens, such as tuberculosis or gonorrhea. This condition is divided into two major categories: acute epididymitis and chronic epididymitis. Due to inflammatory changes, the epididymis may become obstructed, leading to secondary infertility. The symptoms of epididymitis include the following clinical manifestations:
(1) Symptoms of acute epididymitis: Pain, swelling, and redness in the affected scrotum, with pain radiating to the abdomen and the same side of the inner thigh. This often causes patients to bend their waist and clutch their abdomen. Local tenderness in the scrotum, and significant tenderness upon palpation of the epididymis. In severe cases, fever may occur, and suppurative lesions may form around the epididymis.
(2) Symptoms of chronic epididymitis: Pain, swelling, or redness in the affected scrotum, with pain radiating to the abdomen and inner thigh. The clinical symptoms are milder than those of acute epididymitis, with a slow onset and a history of recurrent episodes. It is often accompanied by orchitis and epididymitis. Physical examination may reveal thickening and hardening of the epididymis in the scrotal region. In severe cases, the epididymis may become adhered to surrounding tissues, with tense cremaster muscles and retraction of the scrotum and testes.
Epididymitis that develops due to epididymal damage or after a vasectomy manifests as nodules that spread from the center to both ends of the epididymis. The epididymis may become thickened or adhered, and the nodules may be either painful or asymptomatic.
To improve health, it is advisable to adopt healthier lifestyle habits, enhance physical fitness, quit smoking and alcohol consumption, avoid spicy foods, maintain a regular daily routine, and strengthen nutrition while engaging in moderate physical exercise.
There are many Western medications for treating male infertility, such as:
- Drugs that enhance sperm vitality and prolong their survival time, such as adenosine triphosphate (ATP), arginine, and GnRH-releasing hormone.
- Medications that regulate and stimulate the body's spermatogenesis process, such as clomiphene, tamoxifen, and gonadotropin-releasing hormone.
- Medications for treating reproductive tract infections, mostly antibiotics, which can be selected based on the type and location of the infection.
