Overview: Luteal phase defect refers to insufficient secretion of estrogen and progesterone by the corpus luteum, leading to inadequate secretory changes in the endometrium. Luteal phase defect often results in luteal phase bleeding, impaired implantation of the zygote, infertility, and habitual abortion.
II. Etiology: The maintenance of normal luteal function relies on the proper functioning of the hypothalamic-pituitary-ovarian axis. Abnormalities not only during the luteal phase but also during the follicular phase can lead to luteal dysfunction. Currently, it is generally believed that luteal phase defect is associated with the following factors:
1. Insufficient secretion of FSH during the follicular phase, with low levels of FSH and estradiol in the follicular fluid.
2. Incomplete LH peak during ovulation.
3. Insufficient secretion of LH during the luteal phase or inadequate pulsatile secretion of LH.
4. Abnormal steroid hormone receptors in endometrial cells, leading to a poor response to hormones secreted by the corpus luteum.
