Testicular scrotal manifestation

Patient's question:

Main symptoms: Testicles feel hard and connected to the veins, some of the blood vessels are slightly hard and thick. The scrotum is always cool but not very moist. Sometimes pressing the testicles lightly causes pain. I have a habit of masturbation. This abnormality started after a masturbation session, when I was lying on the bed and was achieved through friction with the bed. Onset date: February 9, 2009. Test results: Testicular ultrasound is normal. Semen analysis shows low sperm motility. Urine analysis shows high uric acid levels. Some other test results are not very normal. I feel that morning erections seem to be less frequent.

Doctor's answer:

Hello: Based on the situation you described, it is likely to be varicocele. Varicocele is a common condition in young and middle-aged men, mostly local discomfort and a heavy sensation, which can radiate to the lower abdomen, groin, and waist. The symptoms worsen after walking or exertion but can be relieved or reduced by rest and lying flat. Varicocele is a vascular mass in the scrotum formed by the dilation and elongation of the pampiniform plexus of veins. It occurs in 95% of cases on the left side, with bilateral cases being less common. It is one of the most common scrotal masses in young people and is also a significant factor or cause of male infertility. In patients with varicocele, sperm count decreases, affecting fertility. After surgical treatment, fertility can be restored. Additionally, some patients may experience symptoms such as mental restlessness, anxiety, insomnia, general fatigue, and impotence. Severe varicocele can lead to atrophy of the affected testicle, prompting patients to seek medical attention due to the testicle's reduced size. It is recommended that you seek further medical examination to confirm the diagnosis and avoid targeted treatment.
