Will growth-restricted type 1 transform into type 2 or type 3?

Patient's question:

Single, pregnancy at 16 weeks? Ultrasound shows a 30% difference between the two babies. Diagnosis: Selective Growth Restriction Type 1. Will it transform into Type 2 or Type 3?

Doctor's answer:

A monochorionic diamniotic (MCDA) twin pregnancy may again present with severe twin-twin transfusion syndrome (TTTS). If the weight difference between the two fetuses in monozygotic twins is >20%, and the weight difference between your babies is greater than 30%, TTTS is already suspected. It is recommended to seek follow-up care at a hospital at the provincial level or higher, where placental laser coagulation may be considered.
Testing fetal size, amniotic fluid levels, and umbilical artery blood flow can help detect complications early.
